Arizona births prior to 1950 require a manual search of records. Please send a fax request to the Arizona Department of Health Services Vital Records Office for requested data prior to 1950. The service is free of charge to state agencies, but customer requests are a higher priority. Arizona requests have a higher priority over out-of-state requests. Turnaround time for customers is approximately 10 days. Turnaround time is subject to change based upon the number of inquiries.

NOTE: If an individual's birth is not recorded with Vital Records within one year of the birth, there is a process required to establish a delayed record of birth. This process is typically completed in 6-12 months.

Multiple records will not be returned from a search. If multiple records are found, additional search criteria must be entered to narrow the result set. The information requested must match the information recorded on the official birth certificate. Possible matches will not be returned, only exact matches. Enter as much information as possible when performing a search to eliminate the possibility of several matches. If you are unable to find a definitive match, please contact the Arizona Department of Health Services Vital Records Office.

Many birth records contain names of Hispanic origin. In some cases, names containing Hispanic characters have been replaced with their English equivalent (e.g. MUNOZ). In other cases, all Hispanic characters have been replaced with a question mark character (e.g. MU?OZ). If you suspect that the name you are searching for may contain Hispanic characters, try using the question mark character in place of each Hispanic character.

Enter the child's middle name as recorded on the birth certificate.

Enter the name of the hospital in which the child was born. The name must match the name recorded on the birth certificate. Data may not be available prior to 1969.

Enter the Mother's/Parent's Last Name prior to First Marriage. Due to the sensitive nature of this data, the Mother's/Parent's last name may be entered to further refine a search, however, it will not be displayed on the printable version of the response.

Enter the Mother's/Parent's first name.

Enter the Mother's/Parent's middle name.

Enter the Mother's/Parent's date of birth. Data may not be available prior to 1989.

Enter the Mother's/Parent's place of birth. If born in the United States, select the State option button and select a valid state from the drop down list. If born outside the United States, select the Country option button and select a valid country from the drop down list.
